Registration
Step 1: Referral
For intake and treatment, you need a referral from your general practitioner for specialized mental healthcare. The general practitioner can send the referral via Zorgdomein. The referral must be received before your first appointment. Referrals for basic mental healthcare are not possible.

A referral is not the same as registering with the practice. To register for the waitlist, go to step 2 below.



Step 2: Register for the waitlist

Closed to new registrations

Are you a new to the practice? Please check if you are insured at:

  • CZ (Nationale Nederlanden, OHRA, Just)
  • ASR (ASR, ASR ik kies zelf)

The practice does not have a contract with these health insurers. Unfortunately, you cannot register as a patient.




Step 3: Intake form

Once you are at the top of the waitlist, you will automatically receive an e-mail from the practice via Zorgmail.

Before you schedule an appointment, it helps if you describe how your complaints started, how they developed, and how severe they are. You can do this using the intake form. To complete it you can use any PDF editor; if you do not have one, you can use Adobe Acrobat Reader for free. You can send your completed intake form as a reply to the Zorgmail message you receive from the practice once you are at the top of the waitlist.

Intake forms that have not been filled in digitally will not be processed. Handwritten forms cannot be processed.



Step 4: Schedule your first appointment

Once your intake form has been processed, you can schedule a first appointment. Schedule your first 45-minute session via the Appointments page. There you can choose a time slot that suits you.



Step 5: Intake phase
The intake can take place in person or via videocall and takes 45 minutes. At the first intake session we will discuss your needs and plan any further intake appointments. Please bring a valid ID (passport, driver's license, or identity card). It is mandatory to record your BSN in the electronic health record. In principle, no treatment advice is given during the intake phase; that follows once the intake phase is complete.



Step 6: Treatment Plan

If together we decide to start medication, then we will also need to monitor its effects and side effects. In some cases, this may also be done via your general practitioner. In addition, we will develop a plan for its use and eventual discontinuation. In some cases, you may be referred to a different therapist with a specific expertise that best suits your requirements. Please also feel free to schedule a check-in appointment to discuss whether any previously made diagnosis (here or elsewhere) remains accurate.
